Shoulder Surgery Rehabilitation | Part 1 | Shoulder Rehab Protocol | Dr. Peter Millett | Colorado


Hello i'm doctor peter millett and welcome to this instructional video
i'm an orthopedic surgeon who specialize in shoulder and sports medicine
we aim to provide the best care for our patients
and part of our job is making sure that you understand the process of healing
and the physical therapy needed to achieve a perfect result
we made this video to help you have a better recovery
an important part of your recovery will be physical therapy
this video will guide you through the various phases of your rehab as you come
back from your injury
you can either watch the video from start to finish which i recommend for
those of you that are about to undergo surgery
or you can simply use the table of contents to review a specific area of

and I'm christy alwine
we're physical therapist at the Howard Head Sports Medicine Center in vail
colorado
together Dirk and i head the shoulder rehabilitation team for doctor peter
millet at the steadman hawkins clinic
in collaboration with doctor millet and the steadman hawkins research foundation
we put together a dvd for you the patients to help you further understand
your shoulder rehabilitation program
the purpose of the video is to serve as a reference for the specific exercises
that we give you in your rehabilitation protocol
you should receive a protocol the day after surgery which details week by week
your rehabilitation progression
the protocol also contains exercises that are specific to the to each phase
of rehabilitation
use the dvd to reference these exercises in addition to the exercise in explained
in the protocol
some of the exercises are also applicable to non-surgical shoulder
injuries
again use the information your therapist gives you in your detailed protocol and then
use the video to further supplement the instructions that your therapist
gives to you
you'll also find that the dvd is divided up into sections so that you can just
reference the specific exercises you'll see many exercises
outlining the rehabilitation protocol
if you referenced the the dvd you'll find that you can just scroll through
specific exercises to find the exercise you'd like to reference at that time

so now i'm going to go over a few of the instructions for changing your post-op
dressing
when you first come down to therapy
you will probably have a very bulky dressing that was applied during surgery
we'll remove that dressing and apply a water resistant dressing that you can
actually wear in the shower
and i'll teach you right now how to change that
so you'll notice right now the
the wound is basically exposed with steri- strips covering it these steri-strips will
remain intact for approximately ten to fourteen days
we'll use the
going to go ahead and apply a pair of gloves here
we'll use this dressing here this is called a op-site
as we remove the op-site dressing
the instructions are you've remove the paper backing first
there are two tabs on either and that are not sticky so you want to just hold
those to make sure you don't touch the white part
place that over
the wound
and you'll then
gently
smooth over the clear plastic part okay
and then you can remove it says number two on here your remove that outer
layer
and there you have a ddressing that can be you can use that in the shower
and
we recommend that you change the dressing everyone one to two days and what
you want to look for is any drainage coming through if you do see drainage go
ahead and remove
the dressing at that point or if it looks like it's gotten wet or steam bubbles
underneath after shower go ahead and change that as well
we're looking at the wound what we want a also examine for every time we take the
dressing off is any signs of infection those signs include redness around the
incision streaking you might feel warmth around incisions to check for that
any drainage that's maybe not bloody so it's ok if there's a little
bit of blood but nothing that's green yellow in color and if you feel
feverish or are having chills be sure to let someone in doctor millet's office
know as soon as possible
immediately after the surgery it is normal to have increased warmth redness
and bruising around the shoulder
this section of the video is to show you how to apply the sling most patients
will be provided
with a sling just like this this is called an abduction pillow
this is to keep your shoulder off your body and to provide the rotator cuff with
optimal
circulation during the rehabilitation process
alright lindsey let's have your just move your arm
out to the outside without actually
using your muscles
now what your going to do is with the left hand
secure that shut
this portion is the portion that goes around the neck so bring this to the back of
your neck
and you want to apply it to these two clips
you are allowed to use your hand for this portion of applying the sling
ok we'll have you stand up now
and behind you let's have you turn this way
you can grab this portion of the belt
this goes around your waist
turn around to the front
and attaches right here
a sling that fits properly
should have your elbow
off to the side of your body so that we provide the the rotator cuff with optimal
circulation during the healing process
the forearm
should be parallel to the ground

Drug Rehab Treatment and Recovery classes -Get clean and Stay clean

 Philip: “So before I came in here, I’d been struggling with drug use on and off for
the last nine years. I had relapsed and had gotten pretty bad the last year and a half
or so, I was using painkillers on a daily basis for several months. Once that ran out
I started using cocaine, not every day but when I did use it would be for maybe five,
six days at a time; with very little sleep, very little food. I lost about thirty pounds,
I was completely miserable; um I called out of work more often than I went to work. life
just felt completely unmanageable. I knew it was falling apart around me but I was powerless
to do anything about it, just couldn’t stop. I ended up in the hospital twice. My friends
came home one night and found me passed out on the floor, barely breathing. they, they
helped me out until the ambulance got there, I was in the hospital overnight, as soon as
I got out I started sniffing cocaine again at nine o’clock in the morning. I was hopeless
basically. I didn’t know where to turn. I knew that there was some better form of
life out there but I had no idea how I could get back in touch with it. And that’s when
I was fortunate enough to come to The Recovery Place, and in that time I’ve definitely
been shown the path back to a healthy, productive, faith-based life; and I’m just grateful
for this opportunity that I’ve had here. It’s really helped me get back in touch
with God and a manner of living that will not kill me.”

Lessons for Healthy Living - Class 1 (excerpt)



The title of this class is: Lions Donít Get Headaches. This is an introductory class that
will help you get an understanding of how your body works. By the time you leave today,
you will know when youíre hungry, what your body is asking for and what will satisfy those
needs. As you probably know, there is a lot of confusion how many of you have ever been
to Barnes & Noble and visited the nutrition or health section? How many books are there?
Hundreds! How many opinions do you think there are? Hundreds and thousands of opinions. By
the time you leave today, you should have a firm understanding of what your body is
asking for and how it works. You will also understand the title of the class, Lions Donít
Get Headaches. How many of you think lions get headaches? We donít know, right?
When weíre making a change, when you make a change in your life, the first thing you
need to do is to learn to listen to your body. You have to learn what your body is saying
and then you have to learn how to respond appropriately. If you donít know what your
body is saying and you donít know how to respond..we know how most of us respond what
do you do when youíre bored? Eat. Itís probably not an appropriate response. You will learn
how to respond appropriately.
The other thing I want to encourage you now that weíre going into a five week series,
is donít feel that you have to make all the changes at once. Everyone in this room is
an adult; youíve been living with yourself a long time. You probably know your personality.
If you donít have a very extreme personality, or a black and white type of personality where
you just do it or you donít do it. then I encourage you to make gradual changes. Donít
think you have to rush into things. Because five weeksí worth of changes is going to
last you only about five weeks. Understand? This is really about lifetime changes or lifestyle
changes. You are much better off taking six months to make changes and staying with those
changes through your entire life than you are rushing into something that you canít
really hold on to. You need to know your own personality. My personality is such that when
I see something that I think is true, I convert overnight. But, not everyone is like that.
I encourage you to make gradual changes if that works best for you.
The first thing you need to learn.for many of you the next couple of minutes will come
as quite a surprise: the first thing you have to do is learn how to feel hungry. Is everyone
here confident that you know what hunger is? What is hunger? Anyone?
Dr. Andrea: You can just say it out loud; we wonít.what do you think hunger is?
Dr. Jeff: You can say it out loud. What is hunger? Now, is anyone here under five years
old? No. Youíve all been eating for a long period of time. Something is driving you to
eat, right? What is that force? When your stomach growls? Thatís right, when your blood
sugar dropsÖanyone else? Youíve all been eating: what drives you to eat? Twelve oíclock,
the time of day? Anyone else? Habit, pizza [laughing]. You see a certain food; pain in
your stomach, smell, mouth waters, advertisements, headaches, cravings, pregnancy[laughing],
what else? What are the other feelings that you get that you associate with hunger? Comfort,
uncomfortable, the shakes, does anyone get weakness or irritability? Anything else?
Let me ask you a question: Do you think when a lion is sitting under a tree in the plains
of Africa that they get the shakes when they get hungry? Or, do they get a headache or
pain in their stomach, get a little moody or a little attitude, or stand in front of
a refrigerator? Mostly this is a manís trait, right? You stand there like this, you look,
you close it, you wait three minutes, you hope something else appears, you open it again,
you say there is nothing else in there and then eventually you either walk away or eat
something youíre not in the mood for.
So, I am going to read a list of the things that I have heard in response to this question.
I have taught a class similar to this for about ten years. This is the list I have compiled
over the years, you will hear some of the things that you included: headache, fatigue,
weakness, moodiness, crankiness, pain in stomach, low blood sugar, dizziness, irritability,
nausea, listless, tired, empty stomach, bored, time of day, habit, cravings, foggy brain,
sick, low energy, acid stomach need something to soothe stomach, and dissatisfied. I have
to tell you: not one single one of those things is hunger!
Those of you who have been following those feelings for thirty, forty, or twenty-five
years or however old you are, youíre following something that is not hunger. Now, if you
came to me (we have a chiropractic practice) and you said, ìDoc, I have this problem.
I feel sick, I feel nauseous, I feel tired all the time, Iím dizzy and I get headaches.î
What would I consider those things? Symptoms! Yet if youíre home and you think itís time
to eat, you consider those things signs of hunger. They are not signs of hunger. They
are signs of sickness.
As a matter of fact, if you want to take note, the least expensive diagnostic test you can
do, you can do it on yourself, it is very safe. All you have to do is to go without
eating for some length of time. For some of you that will be six hours, for some it will
be a day or so depending on. If you get any of these symptoms before you actually get
hungry, there is something wrong with your body.

Basic First Aid & Emergency Medical Treatment : Basic First Aid: The Recovery Position

Hi, my name is Amy, and today we are going to be talking about the recovery position.
Once you have determined that the victim is indeed breathing and they have a pulse, the
safest position for them to be in is on their side that way. If they begin to vomit or aspirate
anything out of their lungs, they are less likely to breathe it in. First thing you want
to do is you are going to roll them away from you. So you want to make sure that the arm
that is on the side furtherest from you is placed above their head gently. That is the
arm that is going to cradle their head. You then bring their leg that is closest to you
up into a kneeling position like so because when you roll them over, this knee is actually
going to catch them from rolling all the way over. Then again, it is always important to
cradle the head, so as you roll them over onto their side, their head is cradled by
their arm, and their knees stop them from rolling all the way over.

Demi: Hello, I'm Demi Lovato.
One of the most exciting things about becoming a young adult
is that you start to have a say in your life.
Where you live, where you work, what you do.
Unfortunately it's hard to feel in control when your life feels out of control.
That's what it felt for me when I was going through my dark period.
I was diagnosed with bipolar disorder. I was hurting myself.
I was anorexic and bulimic, I wasn't doing well.
But it took my parents and my team to pull me aside to
talk to me about getting help.
What I experience may not be exactly what you experience
but for every young adult out there who feels scared or sad
or out of control, who feels like their mental health
diagnosis means that they're unworthy; I want to let you know that you are.
That your life has meaning, you can reach out to someone you trust and
be there for you.
I want you to know that you can overcome and get through anything.
I know where you've been and I know where you're going can be really magical.
